PATIENT ELIGIBILITY CRITERIA

Most terminally ill patients in Limestone County are eligible for hospice care. Patients and families are accepted based on the following criteria:

  • PATIENT IS A RESIDENT OF LIMESTONE COUNTY
  • PATIENT HAS A LIFETIME MEASURE IN WEEKS OR MONTHS, RATHER THAN YEARS
  • PATIENT HAS THE PARTICIPATION OF HIS OR HER PHYSICIAN
  •  

    REFFERALS

    Anyone can refer a patient or family member to Hospice of Limestone County-a friend, family member, physician, nurse, care worker or anyone close enough to care. When we receive a referral, we make a formal assessment to determine if hospice would be the best course of action for the patient’s life.

     

    THE HOSPICE TEAM

    Working closely with the patient’s primary physician, hospice coordinates a team of physicians, nurses, medial specialists and trained volunteers to visit and care for patients in their homes.

    Led by specially trained hospice physicians, hospice teams typically include:

  • Medical Doctor
  • Nurses
  • Chaplain
  • Social Worker
  • Counseling
  • Nutritionist
  • Pharmacist
  • Volunteers
  • Home Health Aide
  • Respite
  • Hospice teams meet regularly to discuss their patient’s needs. They develop a plan of care specific to each patient and continually review it to make sure it meets the changing needs of both the patient and his or her family. As the need for daily care ends, hospice team members continue to offer families Bereavement Care for a period of 13 months.
